I will upload 6 different word document pages that I put in a marketing folder for my practice. I need Letterhead information added to each of these 6 pages. Very easy design. i will close project quickly. 1. at top of page, I need my Name Keeran Kumar, M.D. and credentials underneath. 2. at the top of the page I need the logo with "Interventional Spine and Pain Management" 3. at the bottom of the page I need the address, Phone number and Fax number, email address (emphasis on phone and fax number) 4. I will upload my business card front and back. 5. I will upload the six documents to add letterhead to these documents only. 6. I will also upload the folder cover that has the practice information, logo, etc. 7. Please do not design letterhead only, I need the 6 pages with header and footer incorporated. 8. I do NOT need business cards Thanks so much. 9. Designer may change the marketing word documents, font, color, etc slightly to improve appearance if so desired, to match the Letterhead.
