We would like the following items included in our package: 1. business cards with rounded edges (3.5X2) with three back choices listed below. 2. One business card with straight edges (3.5x4) which is perforated and can be split into 2 business cards, see uploaded template for ideas. 3. Please include stationary, fax paper, email signature, envelopes.
Four back options that we want: 1) appointment reminder on back (at the bottom of the card I want it to read: *Please note that we require at least 24 hours notice for changed appointments. This time is reserved especially for you. Thank you! 2) simple back with logo and website 3) 3rd back with uploaded image of Free teeth whitening (Please replace 'Free teeth whitening' from uploaded image with 'Free Take-Home Whitening System'). 4)On the 3.5x4 business card, the back of one side will be the appointment reminder and the back of the second one will be the uploaded image of the new patient special (Please replace 'free teeth whitening' with: Free Take-Home Whitening System)
