Label for E-Liquid Bottle

We want a new label for our house brand e-liquid. Labels will be used on both 1oz and .5oz glass amber bottles.

With electronic cigarettes and growing fast, we want a label that's fun, colorful, and stands our on the shelf.
Label size is 3" X 1.5" and must include the following. 1. Our Logo 2.WARNING: THIS PRODUCT CONTAINS NICOTINE, A POISONOUS AND ADDICTIVE SUBSTANCE. LOT NUMBER (with space for us to write in up to a 10 digit number) 5. DOB (with space for us to write in the date of manufacturer) 6. PG% 50 VG% 50 (one label should have 50/50 and a 2nd 70/30 7. MG STRENGTH: 0mg, 6mg, 12mg, 18mg, 24mg (there should be a circle beside each strenth for us to color in letting us know the strenth filled in that bottle) 8. Name of Flavor should be dominant. We want this to be easily editable as we will use a common label with all our flavors.
