Please include "Compassionate care for our communtiy" at the bottom of the business card and also in the footer of the stationery.
We want the back of the business card to serve as an appointment reminder. It will need to have a line for Patient's name with "HAS AN APPOINTMENT ON" under this name line. A line for the DATE and time with A.M./P.M. to circle, section for days of the week to circle (MON. TUES. WED. THURS. FRI.). And "PLEASE GIVE 24 HOURS NOTICE IF YOU ARE UNABLE TO KEEP THIS APPOINTMENT."
