(EXPEDITE)Customer Contact Form (simple and modern)

Really clean and beautiful

We wish to update an interface to a newer more visually appealing look. We want the background to be mostly white, it can have texture, but very soft. This is where our clients will log in to to create a support ticket. So we want it really easy to use, and to make them feel like we care.

The layout: (must easily fit on an ipad without resizing, or a small laptop)

You can update the look of the tabs on the top left (the my issues, training, checksheets)

And the home/newticket
We want update the severity 1-5 numbers to look nicer and they should be the first thing you choose.

Then your name (first last)
Then your email address (full email)
Then your cellphone (for texting replies to)
Then your store number or id (to know exact location)

Than a one line description of your problem
Then a larger area to describe your problem in more detail
Then an attachment option
Then a submit button and cancel button

Software

Mostly this is for the fashion industry. It would be a retail store manager, or associate coming to this page.

We want it to be really clean and easy to use.

Mostly a white background, really simple layout, simple to read fonts, easy to understand what your steps are. We are ok with blue colored text in some areas, icon, or whatever you think. I reply very quickly and will comment to tell you if you are on the right track.

    No gradients or shading anywhere.
    Please also demonstrate the pressing of option 1 (by showing red)for the whole button.
    Also please start with a Cap Locks on the first letter for:

    #11 This is a nice concept.
    Please make a color relative to the 1-5 priorities. A very light color on the number. For instance. 5 is Green, 4, is a lighter more yellower green, 3 is yellow, 2 is a yellow with a little red, 1 is red.
    Please make the text "first name" into the text box.
    Please make the text "last name" into the text box.
    Please make the text "email address" into the text box.
    Please make the text and correct "cell phone" to look like this.
    Please make the store number and id info into the text box.
    Please change the text of "title...." to "description of your issue" and put into text box.
    Please change the text "describe...." to "Detailed description" and place that into the text box too.

    For the very top please separate the home / new ticket from the menu bar.

    The menu bar should show colored text when pressed, not the who button. Please do not show a pressed button on anything on this design.

    Please add light blue to the text in the boxes that our clients have to fill in

    Please add a logo onto the page

    Please add buttons to cancel and submit

    Please separate the attachment button and create another text box for that.

    Ok, so I like the blue, but its too much.

    Please make all lines with grays instead of blues. I like the text in the lines being blue.

    Also please put text in the bottom box for "additional details"

    #3 Please remove the blue area, it was just supposed to be a blue line around the support form, but white as the background everywhere.

    Also this is a smaller version, it doesn't allow me to check on an iPad without being blurry. #2 #1 are large enough.

    Please remove the company name area.

    The alignment of the text in the buttons on top is off. (my issues, training, and checksheets)
    Please make the tabs on top without buttons around them (these are actually links)
    I like the idea with the select box for priority issues, but it needs to have a description like we had before.
    I think maybe you could try a white box for the entire form, with light blue lines around it, and white around the box.

    I'm not fan of the gray background.
    Please get rid of all shadows.
    I don't like the logo with transparency. I love it bright.
    I am not fan of the dark blue and light blue mixed.
    I would like the submit and cancel to be color coded.
    We need to add the priority levels 1-5 as the first step.(this is determined first because we then know how important the issue is)
    Please remove the numbers 1.,2.3.,4.,5. because they can assume what order to fill out their name, email, ect.
